Click this tab or use the Keyboard shortcut Alt+I to create or retrieve patient records.
Data entry fields have a white background, calculated fields which cannot be edited have a gray background. Shortcut: Tab between fields instead of clicking the mouse on every input box.
Change to edit or add mode While in Browse mode you may not make changes to the displayed patient data.
Saving the patient record
Patient Name Enter the patient's name. The required format is LastName, FirstName (comma required).
Attending phys Enter the attending physician's name.
Med Record # Enter the patient's medical record number. Please note: to maintain data integrity, use the MRN instead of a financial or encounter number. The MRN is permanently associated with a patient whereas the financial number changes from one admission to the next.
Location Enter the patient's room number (this field may be left blank).
Date of birth Enter the patient's date of birth, in 4-digit year format. Please note: if you experience problems entering dates, see the Getting Started section of the help file for more information.
Tip: Use the left and right arrow keys to move between day, month and year entry.
Or you may enter age instead of birth date by Right clicking on the DOB field. Please note: the database stores the birth date, not age, so to maintain data integrity please use the birth date unless you are unable to obtain this data.
Age On the setup window you may choose to enter the patient's age instead of date of birth. Please note: the database stores the birth date, not age, so to maintain data integrity please use the birth date unless you are unable to obtain this data.
Height Enter the patient's height. The default height unit (cm or inches) is chosen from the Setup window. Or you may enter by the other unit of measure by Right clicking on the height field.
Weight Enter the patient's weight,. The default weight unit (kg or pounds) is chosen from the Setup window. Or you may enter by the other unit of measure by Right clicking on the height field.
Gender Click male or female. Or use the keyboard shortcuts Y for male, X for female.
Other factors Click all of the factors that apply to this patient.
Serum creatinine Enter the most recent serum creatinine and date it was drawn. Shortcut: Press the "T" key while in the date field to automatically enter today's date.
Enter a previous serum creatinine if available. Shortcut: Click the "Add" button in the creatinine frame to copy the most recent creatinine to the previous creatinine and to allow for new input.
Calculated data The patient's creatinine clearance is automatically calculated but may be overridden if, for example, the patient has a measured creatinine clearance. The dosing weight is calculated after a model is selected, it too may be manually overridden. Remember the visual clue for an editable field is a white background.
Patient data limits The following limits are used to determine acceptable patient data. Entry of patient data outside these limits will trigger a warning that the data is unacceptable and will not be saved. Age Between 12 and 110 years Height Between 122 and 213 cm (48 and 84 inches) Weight Between 20 and 250 kg (45 and 550 pounds) Creatinine Between 0.4 and 21 mg% (30 and 1600 SI units)
